APPLICATION PROCEDURES
(Deadline is January 15)

To be eligible, applicants must:

• Possess a Doctor of Pharmacy degree from an accredited school/college of pharmacy
• Possess a license or become licensed by the Pennsylvania State Board of Pharmacy during the residency.
• Participate in an on-site interview at WellSpan Health.

For PGY2 applicants

• Completion of a Pharmacy Practice Residency Program.
• Possess a license or become licensed by the Pennsylvania State Board of Pharmacy during the residency.
• Participate in an on-site interview at WellSpan Health.

When applying, please enclose the following

• Residency application form
• Curriculum vitae
• Transcript
• Three letters of recommendation submitted directly to WellSpan Health
• Resident candidate letter of interest
• Documentation of class rank
