York Hospital Pharmacy Practice Residency Program

Flexible and progressive practiceYork Hospital's Pharmacy Practice Residency Program offers flexibility and progressive practice experiences. An individualized program whose preceptors are experienced and dedicated practitioners, York Hospitals’ program has been designed to ensure competence, build confidence and foster independence.

Our pharmacy services include 24-hour automated and computerized drug distribution, unit-dose, IV admixture, decentralized distribution, creatinine clearance adjustment  dosing program, pharmacokinetic monitoring and dosing service, drug therapy monitoring, and specialized clinical consult services including an anticoagulation management service and a nutrition support service.

The Pharmacy Practice Residency is a one-year program.  We were originally accredited by ASHP in 1999 with subsequent reaccreditation in 2008. Emphasis is placed on providing rational and cost-effective drug therapy in a community hospital setting.  This is accomplished through the provision of clinical pharmacy services. Our goal is to provide an opportunity for professional growth.

PURPOSE
The Purpose of the Post Graduate Year One Pharmacy Practice Residency Program at York Hospital is to provide training in Health System based pharmacy practice and to develop pharmacists to be able to meet the ever changing needs of Health System Pharmacy Practice.  Graduates will be prepared to enter practice as pharmacist clinicians for patient care positions, adjunct faculty positions, or for PGY2 training in area of their choice.

Upon completion of this residency program, pharmacists will be assertive, ethical professionals with a commitment to pharmaceutical care and the profession of pharmacy. These pharmacists will effectively manage and lead change by overcoming the challenges that face pharmacy. They will be skilled providers of pharmaceutical care, educators of patients and health care professionals on drug related topics. They will be valued members of a healthcare team; self-starters who take personal responsibility for achieving optimal patient outcomes by monitoring and documenting their impact on patient care.

SUCCESSFUL COMPLETION
Residents completing the following program requirements will be awarded a PGY1 residency certificate:

  • Successful completion of all core rotations and competencies
  • Demonstrate the ability to work independently for pharmacy-related consults (PK, TPN, Anticoagulation)
  • Develop/revise/evaluate, educate, and implement evidence-based protocol
  • Complete a formal write up of four drug information questions and present two at Pharmacy Department staff meetings
  • Present project at ASHP Midyear, Eastern States, and Resident Research Day
  • Completion of one CE-accredited in-service for the pharmacy staff
  • Completion of 4 pharmacy forum articles and serve as editor for 3 months
  • Complete at least one quality improvement project within the pharmacy department impacting the distributive functions of the pharmacy department.
  • Development of a manuscript and choose a journal appropriate for submission of the practice project or research project
  • Completion of all residency-related documentation (ResiTrak, residency portfolio, and IRB closeout)

ROTATIONS
The pharmacy practice residency program offers a wide variety of patient care activities with the ultimate goal of providing experiential learning in acute and ambulatory patient care, drug information and management. The Pharmacy Practice Residency Program provides advanced training in clinical services including pharmacokinetics, ambulatory care, critical care, family practice, internal medicine, and drug information. Residents will develop skills while providing consultations in areas of pharmacology, pharmacokinetics, toxicology, and disease state management, along with educational programs to the pharmacy (staff and students), nursing, and medical staff; including medical residents, interns, and students.
