York Hospital Pharmacy Critical Care Residency

York Hospital's Pharmacy Critical Care Residency Program offers the opportunity to develop specialty pharmaceutical care skills in both medical and surgical intensive care unit patients. It is precepted by experienced and dedicated practitioners to ensure competence, build confidence and foster independence in the setting of complex drug therapy decision making.

York Hospital has a total of 60 intensive care unit beds. These units Include a medical ICU, level 1 trauma center and ICU, coronary care unit, and 3 transitional units. Interdisciplinary rounds with medical residents, surgery residents, and ICU attendings create an ideal environment for pharmacy involvement and patient care activities..

The Pharmacy Critical Care Residency is a one-year program originally accredited by ASHP in 2001 with subsequent reaccreditation in 2008.  The emphasis of our program is on providing rational and cost-effective drug therapy in a community hospital through the provision of clinical pharmacy services in the ICU setting. Our goal is to provide opportunity for professional growth and the development of a Clinical Pharmacy Specialist in Critical Care.

Upon completion of this residency program, pharmacists will be assertive, ethical professionals with a commitment to pharmaceutical care and the profession of pharmacy. These pharmacists will effectively manage and lead change by overcoming the challenges that face pharmacy. They will be skilled providers of pharmaceutical care and educators of patients and health care professionals on drug related topics. They will become content experts, valued members of a healthcare team; self-starters who take personal responsibility for achieving optimal patient outcomes by monitoring and documenting their impact on patient care.

PURPOSE
The Purpose of the Post Graduate Year Two Critical Care Pharmacy Practice Residency Program at York Hospital is to provide advanced training in Health System based critical care practice and to refine knowledge and skills needed for interdisciplinary practice in the critical care setting.  Graduates will be prepared to enter practice as a clinical pharmacy specialist providing direct patient care, or as a faculty member practicing in a critical care area of their choice.

SUCCESSFUL COMPLETION
Critical Care Pharmacy Residents completing the following program requirements will be awarded a PGY2 residency certificate:

  • Successful completion of all core rotations and competencies
  • Demonstrate the ability to work independently for pharmacy-related consults (PK, TPN, Anticoagulation)
  • Develop/revise/evaluate, educate, and implement evidence-based protocol
  • Participate in at least one Root Cause Analysis (RCA)
  • Present project at ASHP Midyear, Eastern States, and Resident Research Day
  • Completion of one CE-accredited inservice for the pharmacy staff
  • Completion of 4 pharmacy forum articles and serve as editor for 3 months
  • Complete at least one quality improvement project within the pharmacy department impacting the distributive functions of the pharmacy department to the critical care setting.
  • Development of a manuscript and choose a journal appropriate for submission of the practice project or research project
  • Completion of all residency-related documentation (ResiTrak, Residency portfolio, and IRB closeout)

ROTATIONS
The critical care pharmacy residency program, offers the opportunity to build upon skills learned in a pharmacy practice residency with the ultimate goal of providing experiential learning in medical and surgical intensive care units. The critical care residency provides advanced training in critical care pharmacokinetics, drug information, infectious disease, medical critical care and surgical critical care. In addition to practicing in the critical care setting, residents will also have the opportunity to provide educational programs to pharmacy staff and students, nursing and the medical staff (residents, interns and students). The critical care resident will also serve as a role model and leader for pharmacy practice residents.

Rotation Summary
Mandatory:

  • Distributive orientation (1 month) (not required if former PGY1 resident at York Hospital)
  • Pharmacokinetics (2 weeks) (not required if former PGY1 resident at York Hospital)
  • Medical ICU (2 months)
  • Trauma ICU (2 months)
  • Infectious disease (1 month)
  • Emergency medicine (2 months)
  • Cardiothoracic surgery (1 month) 

The following is a required longitudinal rotation:

  • Distributive rotation

Electives:*
Any of the mandatory rotations and:

  • Pain management
  • Palliative care

*Other medical subspecialties by special arrangement
